Covid-19 Vaccination Certificate: How to Generate DOH VaxCert Online in 4 Easy Steps

On September 6, 2021, Department of Health (PH) made a soft launch of the VaxCertPH website. The initial phase of implementation of the VaxCertPH prioritized requests coming from Overseas Filipino Workers (OFWs) and Filipinos who will be departing from the Philippines for international travel with place of residence indicating National Capital Region and Baguio City.

Just recently, the VaxCertPH has opened to the general public and for other purposes based on the recommendations of the VaxCertPH Sub-Technical Working Group to the IATF. Through this regional launch, one can now generate his/her own vaccination certificate conveniently.

Here’s a simple guide how to generate a VaxCert online in 4 easy steps.

STEP 1: Go to https://vaxcert.doh.gov.ph/ and review Data Privacy

The landing page of the site will ask a question on whether or not you’re an international traveler or OFW and if you’re fully vaccinated and you’ll be traveling outside the Philippines in the next two weeks. Before you answer “YES” to proceed, please read carefully the privacy notice.

If you are satisfied that your personal data will be used and stored in the VaxCertPH database and give consent for the processing and other purpose, then by all means, proceed to step 2.

STEP 2: Fill up Personal Information and Vaccination Details

Enter your personal information like Name and Birthdate and make sure they match the details of your vaccination card. Click Next.

Input the details of your first and second dose vaccination such as location and date. Then, click verify vaccination information.

STEP 3: Review Information

Take a last look at the details for accuracy. Click continue if you are done reviewing the details, otherwise, click go back.

STEP 4: Generate CertificateIf your vaccination record is in the system, it will display “Record Found!”. If it says otherwise, you may send an email to vaxcertsupport@doh.gov.ph to request for it (also provide the reference number).

Input your passport number in the field and tick the box if you intend to use the certificate for travel purposes. You may now generate the vaccination certificate. It looks like this:

Congratulations! You may now utilize your Vax Cert when traveling domestically or internationally. Keep a digital copy or print it for hard evidence, just in case it will be required for submission or inspection.
